Clinical Manager – Full time
Job Details: Incumbent must be knowledgeable of Medicare Regulations & JCAHO standards, possess excellent communication skills, ability to manage the patient’s transitions across the continuum of care, and to assist patients or population in achieving or maintaining an optimal level of health. Bachelor’s degree in Nursing is required. Competitive salary.
Intake Coordinator- Full time
This position is responsible for collecting and coordinating all necessary demographic and clinical information necessary to qualify and complete the admission process of patients. Incumbent must be an LPN, currently licensed in the state of the agency operation. Computer skills required. Competitive hourly rates and benefits.
Licensed Clinical Social Worker – Full time
Major duties include, but not limited to the following:
• Crisis Intervention
• Initial bio-psychosocial intake assessments
• Diagnosis and development of treatment plans
• Individual group or family psychotherapy treatment
Requirements: Minimum of five (5) years experience in clinical counseling. Ability to work eight hours per day and five days per week. Competitive hourly rates and benefits.
