Position Summary:
The Care Coordinator is a licensed RN or LPN, who routinely exercises independent discretion and judgment in overseeing the clinical care of residents and household (unit) administrative duties. The Care Coordinator customarily and regularly directs the work of a team of Resident Care Associates (CNA\'s) assigned to a specific household.
The Care Coordinator will primarily be assigned to manage a household(s) of 27 (1st & 2nd shift) or 54 (3rd shift) residents. The Care Coordinator will report directly to the Villa Leader (Nurse Manager).

Requirements
Minimum Experience: 1+ years of experience in a long-term care setting.
Minimum Education: Graduation from a Commonwealth-approved School of Nursing.
Licensure/Certification: Current Pennsylvania RN/LPN Licensure.
